Enclosed are our patient registration forms. Please complete these forms and bring them with you on the day of your appointment. We will also need a copy of your insurance cards and photo ID. Please arrive 15 minutes prior to your appointment.

Your First Visit

Check In

Upon arriving at our practice, we will check you in, verify your insurance and have you complete any necessary forms.

X-Rays & Cleaning

A dental hygienist will take your x-rays, clean your teeth, and speak with you about any concerns you have regarding your smile.

Meet with our Dentists

After your cleaning, Dr. Lockhart or Dr. Nichols will speak with you and the dental hygienist about your cleaning and any concerns you may have. She will provide diagnoses if applicable and discuss any necessary care and treatment.

Next Steps

Following your time with Dr. Lockhart, we will go over any next steps and schedule any necessary follow-up appointments. If there are no follow-up visits needed, we can get you scheduled for your next cleaning.

Lockhart Dental accepts assignments from most dental insurance companies and are happy to file with yours. At your first visit, we will verify your insurance information and determine a copay amount (if applicable). Some plans require payment upfront, followed by a reimbursement to the patient from the insurance provider. As insurance is a contract between the patient and the insurance company, Lockhart Dental does not track or verify your benefits. We strongly encourage you to contact your insurance company and familiarize yourself with the specifics of your plan.

Please be advised that payment is due at the time of service. This includes ALL co-payments, deductibles and full payment if you do not have Dental insurance. If you are unable to pay all at once, we offer financing options.

Dr. Lockhart recommends treatment based on maintaining your optimal dental health, and we would never recommend treatment based on insurance benefits and maximums. Contact us if you have any questions. We would be happy to work with you to ensure you can receive the dental care you need.
